The NFC Participation Waiver is needed for each player on the travel team.   The attached form has been provided to each parent as a part of registration.  The parent must accept to register.  The copies of the release are kept by North FC.

We suggest keeping a couple "blank" copies with your player cards.  This way if you need a parent to complete the form, you have one to provide.
